Click on the link. When the box opens, click save, and save to the desktop titled “Member School Name - Rule Book Order Form”. Fill out the document, save it, and email as an attachment per the instructions in the file. If you have Microsoft Excel 2007, make sure to save it as a 97-03 workbook.
